If you are interested in a Sunday appointment please call us in advance.
For whatever reason you may need to cancel an appointment we would appreciate it if you contact our office as soon as possible. We require at least a 48-hour notice in advance for broken appointments fee may apply. We make a point of contacting our patients at the number that has been provided to remind/confirm appointments. If we do not hear back form you by the evening before your appointment, we will assume you will not be present for your appointment.
There are a total of seven new-patient forms that we ask to be completed before your visit in order to serve you more efficiently. If you are an insurance holder, please bring your dental insurance card so that we may obtain a copy for our records. Thank you for your cooperation.
